Dependent Coverage

  • Medical and Prescription:
    Dependents are covered up until the end of the year they turn 26 years old. NO verification is needed to continue medical and prescription coverage up to age 26.

    Dental and Vision:
    Dependents are covered for dental and vision up until the end of the year they turn 19. After that, Full Time Student Verification is required to prove dependent eligible status.  Dependents age 19-23 who are attending college and have a minimum of 12 credits per semester must provide at least one of the following :

    • Registrar’s Letter
    • Copy of Schedule (showing 12+CREDITS)
    • Copy of Tuition Payment

    Verification must be turned in to the Benefits Department in Human Resources. Use the following verification checklist to ensure you turn in the necessary information:  Verification Checklist

    By providing us with a copy of one of the above we can submit these forms on your behalf, as well as have proof for any claims that may come as a result. We are happy to provide this service for our employees when we have this information on file. 

    Chapter 375 Coverage up to Age 31

    Dependents under age 31 who would otherwise lose coverage due to an age limit under their parent’s group health plan (age-out) will be permitted to elect continued coverage if they meet certain eligibility requirements.  Chapter 375 Fact Sheet and Overage Dependents Eligibilty Information

    This coverage is available up to age 31. Fill out this enrollment forms to continue coverage for your dependents when they no longer qualify for coverage under your medical plan.   Enrollment Form for up to age 31
