Application and Request for Payment of Transportation Aid

The following form must be completed and emailed back to Heidi Bermudez (hbermudez@pemb.org) in order to apply for Private School Transportation.
Requests for payment of Transportation Aid must be completed TWICE each year.
For the first semester of the year (September to January), please complete the form by December 30th.
For the second semester of the year (January to June), please complete the form by this upcoming June 30th.
Please print out one of the forms below, fill it out and email it back to Heidi Bermudez (hbermudez@pemb.org).
