Hornet families, 

Any student interested in playing a fall sport at Helen Fort or Newcomb (Boys' Soccer, Girls' Soccer, Cross Country, Field Hockey, or Flag Football) must have a physical submitted to the nurse's office by Friday, August 25, 2023. If they do not, they cannot participate in fall sports.  Physical packets are available in the nurse's office and on our school website.

This year there is a change to the required athletic paperwork.  NOTHING HAS CHANGED WITH THE PHYSICAL PAPERWORK.  YOU STILL NEED TO SUBMIT A PAPER PHYSICAL PACKET TO THE SCHOOL NURSE THAT IS COMPLETED BY YOUR DOCTOR.  

For the rest of the paperwork, we have migrated from paper signatures for the athletic waivers to digital signatures through the Genesis portal.  Pemberton Township High School started this last year and it was a success.  If you would like your child to participate in middle school sports, you MUST log in to your Genesis account, then click on the FORMS tab and digitally sign/update the 5 forms that are listed:

Health History  

Once these are all digitally signed (which now you only have to complete once a year) AND your child has submitted a physical packet from your doctor that was approved by our school doctor, then you are eligible to participate in athletics at Helen Fort and Newcomb. Fall sports start on Monday, September 18^th so please get these signed by then. Thank you for your patience and flexibility as we transition to digital signatures for athletics waivers.
