Welcome to Stackhouse School Kidergarteners!
REQUIRERMENTS FOR KINDERGARTEN:
Physical Exam & Health Questionnaire Completed
State of NJ Mandated Immunizations
4 doses DTP (Diphtheria, Tetanus, Pertussis) with 1 dose given after 4th birthday or 5 doses
3 doses Polio (OPV) with 1 dose given after the 4th birthday or any 4 doses
2 doses of MMR (Measles, Mumps, Rubella)
1 dose of Varivax/Varicella (Chicken Pox) dose must be administered on or after 1st birthday
3 doses of Hepatitis B (series of 3 injections at specific intervals)
It is always helpful to have an extra set of clothes available at school for any bathroom accidents.
Please send a set in that is season appropriate and labeled with your child's name on the zip lock bag.
All the above should be received on the first day of school. Thank you for your cooperation!
